Patients agree and acknowledge that should they opt to go outside of their insurance, that they are financially responsible for all fees for service and that payment for such fees is due in full at the time of service. Further they understand that by electing to pay the self-pay rate that Own Sleep Medicine Services, LLC will not process claims on their behalf with any insurance. Therefore, they are financially responsible for services rendered, even if they discontinue the use of their equipment at any point in the future.

NEW PATIENT TELEMEDICINE CONSULTATION / FOLLOW UP
Patients are financially responsible for services rendered, regardless of the outcome of the consultation and regardless if they elect to move forward with testing or treatment now or in the future.

HOME SLEEP TESTING
Patients are financially responsible for services rendered, even if they knowingly or unknowingly do not complete the testing on the dates they have a testing device and are scheduled to complete the Home Sleep Test.
