• Is valid only for one (1) swap per patient.

• Is valid only for patients who received their AutoCPAP Setup through Own Sleep.

• Is valid only for the first sixty (60) days after AutoCPAP Setup is confirmed delivered.

A flat-rate shipping fee may apply.
